PARENT CONSENT, WAIVER OF LIABILITY AND MEDICAL RELEASE - New Please submit ASAP

4.1 Entry Deadline: Entry application, including all fees, must be received by October 10,
2008. ISSA will begin reallocating open spots on Oct. 11.
4.2 Eligible competitors must enter by completing an official entry form online and payment by check. Online registration will open on Sept 15th.

4.3 Competitors and parents must complete the PARENT CONSENT, WAIVER OF LIABILITY AND MEDICAL RELEASE prior to competition. It is recommended that they be submitted by mail, and that the competitor bring a copy with them the day of the event.

5. ENTRY FEE
5.1 The entry fee is $110 per competitor. This fee includes two continental breakfasts, three box
lunches and a dinner.
5.2 Entry fees will be paid by check by mail. Please mail check to:

5.3 Meal packages are also available for Coaches and Parents for $60.00 and must be indicated and paid with the entry application.

12.1 No private coach or spectator boats will be permitted. One coach/support person per team will be permitted on coach boat(s) provided by the organizing authority. All spectators and other support personnel will be permitted to view the racing from the shore and sailing center docks.
